including complete address information
- Emergency

- Insurance Cards
- Name of Referring Physician

have any changes of address or insurance information,

We can update your information in a matter

For each visit to The Heart Center
Medical Group, all patients must register at our
main registration
desk, conveniently located
just
inside the doors
of the main entrance. Our staff will electronically notify our
medical staff of
